The purpose of this worksheet is to simply get you thinking about what you really, really want…and why you want it.
Write what comes to you first…don’t allow doubt or fear to creep in…write your desires down as if no one was watching or if no one else was going to read them!
1. What are at least 3 changes that I want to make in my life during the next 12 months?
2. Why is it so important to me that I make these changes? What will it cost me if I don’t make do it? (Personally and Professionally)
3. What has gotten in the way of me achieving this up until now? What keeps getting in the way?
4. Name one critical action you must make that will make a significant step towards achieving what you want. (You can add more if you wish).
5. Name the date when you will take this action(s). Who can support you in making this commitment? A buddy? A group?
Now….Just do it.
